Please provide the following information about your ThayerRags™ Custom Doo-rag in an email to the address below.

For each doo-rag: (Items with a  * are needed for each different doo-rag)

  1. 3-digit Order Number *

  2.  Size * (REG, SM, XS, XXS, Newborn)

  3.  Quantity *

  4. Rear Closure Type * (526 Tie or 520 Hook & Loop)

  5. Embroidery Text ("Name" to be embroidered on band.) [Omit if no embroidery.]

  6. Font Style Number (see below) [Omit if no embroidery.]

  7. Embroidery Thread Color [Omit if no embroidery.]

  8. Foam Visor Color [Omit if no visor.]

 

For every email message: (Items with a ** are needed only once for every email)

  1. Your Name ** (Person submitting information.)

  2. Your Email address ** (For return contact.)

  3. Your Zipcode or Country Code ** (For calculating shipping.)

  4. Additional Information (Any additional information that you think we may need.)
